OK, so how about this scenario (for both Mac and Windows, except where  
noted):

1. User double-clicks app icon, or launches it from Start menu, Dock,  
or wherever.
    (a) No (findable) documents were open last time it quit:
        Open an untitled plan document.
    (b) One or more (findable) plan documents were open last time it  
quit:
        Open those documents.
2. User double-clicks plan document/s to open it/them.
    (a) App already running:
        Open those documents (in addition to any already open).
    (b) App not running:
        Open the app. Load the double-clicked document/s. Forget any  
documents previously opened.
3. User quits from app, via File->Exit (File->Quit on Mac).
    Any open plan documents are remembered for 1b above. App quits.

4. User closes last plan document window in running app.
    (a) Mac:
        Nothing. Mac continues to display the menu bar. This is  
standard behaviour.
    (b) Windows:
        App quits.
        Option (in preferences): Display the Open/Create dialog.

Assuming everyone is on-board with this, then the only decision would  
be whether to make the option in 4b on or off by default.

What I could do, is make it on by default, and have a checkbox on that  
dialog: "Don't show this dialog again. Always quit the application" to  
switch it off.

Since the Catalogue Manager no longer requires all plan documents to  
be closed, I can streamline the dialog a bit as well by removing that  
button.
