I'm pretty sure I ran a2 at least once. The splash screen is there and gone so fast, I 
can't ever read it.

As an experiment, I started a second instance and I got the 'multiple instances not 
allowed' dialog.  I dismissed that, then the first dialog you sent of picture of above 
popped up. I selected 'create new' and then the app opened, but an error message appeared:
"An exception of class NilObjectException was not handled. The application must shut 
down." Two of these popped up one after the other (both still visible).

I don't know if this unhandled exception is just an artifact of running two instances, 
or if it has anything to do with the original problem.

Note that the original instance remains catatonic through all of this, it's the second 
instance (based on monitoring task manager) that's popping up the screens and boxes.
